Job Opportunity

We are seeking a highly skilled Registered Nurse to join our team as a Clinical Care Manager for our Medical Assistant Treatment Program (MAT) in Worcester, MA.

The successful candidate will provide evidence-based care management to our MAT patients, ensuring they receive optimal care, education on lifestyle changes, and preventive care across multiple settings.

Key Responsibilities

  • Coordinate and manage the care of MAT program patients, working closely with Primary Care Providers to ensure seamless care transitions.
  • Develop and implement care plans to prevent disease exacerbation, improve outcomes, and reduce hospital and ER visits.
  • Communicate effectively with the care team to ensure patient needs are met and care plans are executed.
  • Provide direct patient care, including assessments, vaccinations, medication reconciliation, and lab reviews.
  • Collaborate with patients and support systems to create self-management plans and document patient care and progress in the EHR.

Requirements

  • Registered Nurse License in MA
  • Bachelor's degree in Nursing from an accredited college or university
  • 1-2 years of clinical experience
  • Current CPR certification
  • COVID-19 booster and yearly flu vaccine required
