Avera@Home Sioux Falls is seeking a dedicated and compassionate Registered Nurse to join our team as a Patient Care Coordinator.
About the Role:
- Coordinate patient care services between hospital settings and home settings.
- Promote, educate, and coordinate services for patients transitioning from hospital to home healthcare or hospice services.
Responsibilities:
- Develop and implement individualized care plans for patients.
- Collaborate with healthcare teams to ensure seamless transitions and continuity of care.
- Provide education and support to patients and their families on disease management and self-care.
- Coordinate with community resources to ensure patients receive necessary services and support.
Requirements:
- Bachelor's degree in nursing preferred.
- Graduate from a CCNE or NLN accredited nursing program is strongly preferred.
- Minimum 2 years home health/hospice related experience and/or training preferred.
- Sound knowledge of community resources preferred.
Licensure/Certification/Registration:
- License as a Registered Nurse in the State of South Dakota and/or meeting the requirements of Multi-state licensure is required.
- Licensed as a Registered Nurse in the State of Minnesota may be required depending on the service area.
- Integrated Care Management Model Certification is preferred.
Work Schedule:
- 8:00 am - 4:30 pm as needed; PRN.
Pay:
The hourly range for this position is listed below. Actual hourly rate dependent upon experience.
- $44.25 per hour.
