Clinical Nurse Coordinator

Montefiore Medical Center is seeking a highly skilled Clinical Nurse Coordinator to join our team. As a key member of our nursing staff, you will be responsible for coordinating and implementing patient care services for a designated unit or patient population.
Responsibilities:- Coordinate and implement patient care services, ensuring high-quality care and excellent patient outcomes.
- Collaborate with healthcare teams to develop and implement care plans, addressing the unique needs of each patient.
- Monitor and evaluate patient care, identifying areas for improvement and implementing changes as needed.
- Provide education and support to patients and families, promoting health and wellness.
- Participate in performance improvement initiatives, contributing to the development of best practices and policies.
- New York State License for Registered Professional Nurse and current registration or valid permit to practice in New York State as an R.N.
- Certification preferred in specialty or efforts to achieve certification.
- BSN degree required and 5 years clinical experience, two of which must be in the specialty or related service.
- Demonstrates clinical leadership, problem-solving skills, competency, and principles of Montefiore Excellence.
