MDS Assessment Coordinator

Job Summary
We are seeking an experienced RN MDS Nurse to join our team at Chamberlin Park. As an RN MDS Nurse, you will play a critical role in ensuring the delivery of high-quality patient care and achieving compliance with federal and state regulations.
Key Responsibilities
This role will involve:
- Coordinating the RAI process to ensure accurate and timely completion of MDS assessments
- Providing Medicare, Medicaid, and managed care oversight to ensure reimbursement is received for each resident
- Collaborating with the healthcare team to achieve compliance with federal and state regulations
Requirements
To be successful in this role, you will need:
- A valid RN license in the state employed
- Three years of experience in a long term care environment preferred
- Experience with the MDS/RAI process and/or case management preferred
