About the Role:
The MDS Nurse works closely with our clinical leadership team to complete assessments and care plans for our residents. This involves conducting physical assessments, interviewing residents, and reviewing medical information from hospitals and outside agencies. You will also discuss resident care needs with caregivers, including physicians, nurses, social workers, therapists, dietitians, and activity staff.

Responsibilities:
* Assess residents through physical examination, interview, and chart review
* Complete MDS, CAA's, and care plans within regulated timeframes
* Review information from hospitals, consults, and outside agencies
* Discuss resident care needs with caregivers
* Conduct PPS meetings and maintain MDS assessments according to Medicare schedules

Requirements:
* High school graduate or equivalent
* Strong knowledge of the Resident Assessment Instrument (RAI) process and Prospective Payment Process (PPS)
* Excellent communication and interpersonal skills
* Ability to work effectively in a fast-paced environment
