Job Summary

CenterWell Home Health is seeking a skilled Nursing Case Manager to provide high-quality patient care in a home health setting. As a member of our team, you will be responsible for coordinating patient care, administering ongoing care and case management, and ensuring seamless transitions between healthcare professionals.

Key Responsibilities
  • Conduct thorough assessments of patients' needs and develop individualized care plans
  • Collaborate with healthcare teams to ensure timely and effective care delivery
  • Communicate regularly with patients, families, and caregivers to ensure understanding and satisfaction with care services
Qualifications
  • Diploma, Associate, or Bachelor Degree in Nursing
  • A minimum of one year of nursing experience, preferably in med surg, ICU, ER, or acute settings
  • Strong analytical and problem-solving skills
