Job Summary

We are seeking a skilled Case Manager II to join our team at Telecare Corporation. As a primary support and case manager, you will work with individuals with mental illness and other co-occurring disorders to help them live independently in their chosen community.

Key Responsibilities
  • Provide direct supervision and support to individuals with mental illness and other co-occurring disorders
  • Assist clients in achieving their goals and living independently in their community
  • Develop and implement individualized treatment plans to meet the unique needs of each client
  • Collaborate with other healthcare professionals to ensure comprehensive care
  • Provide crisis intervention and support as needed
Requirements
  • High School Graduation or G.E.D. equivalent and two (2) years of direct services in Mental Health or community services
  • Valid and current driver's license, and personal vehicle insurance with your name listed as a driver
  • Willingness to use your personal vehicle to drive clients to appointments and groups, etc.
  • Bilingual Spanish
