About the Role

We are seeking a highly skilled and compassionate Wound Care Nurse to join our team at Providence Home and Community Care. As a key member of our interdisciplinary team, you will provide high-quality, patient-centered care to individuals in need of wound care services.

Key Responsibilities
  • Assess and develop individualized care plans for patients with complex wound care needs
  • Provide wound care services, including debridement, dressing changes, and wound assessment
  • Collaborate with other healthcare professionals to ensure comprehensive care and coordination
  • Communicate effectively with patients, families, and caregivers to ensure understanding and adherence to care plans
  • Participate in ongoing education and training to stay current with best practices and evidence-based guidelines
Requirements
  • Graduate of an accredited school of nursing
  • Current Washington Registered Nurse License
  • National Provider BLS - American Heart Association certification
  • National Provider Identifier (NPI) number
  • 2 years of experience practicing as a registered nurse in a Long Term Care, Medical/Surgical Unit, and/or Skilled Nursing Facility
  • 1 year of recent, hands-on technical clinical experience in a Medicare-certified Homecare or Hospice agency/facility doing intermittent visits and case management
  • Recent hands-on experience in wound care, venipuncture, ostomy, catheters, and feeding tubes
  • Rehab nursing experience
  • Experience working with an EMR system
  • Experience with office software programs including Outlook, Word, and/or Excel
