Home Health RN Clinical Manager

Are you a dedicated and compassionate nurse looking for a challenging role in home health care? We have an exciting opportunity for a Home Health RN Clinical Manager to join our team at HCA Florida Healthcare at Home.
About the RoleAs a Home Health RN Clinical Manager, you will be responsible for supervising and coordinating the delivery of quality interdisciplinary home health care services. You will ensure compliance with federal and state standards, as well as clinical best practices. Your expertise will be essential in developing and implementing individualized care plans that meet the unique needs of our patients.
Key Responsibilities- Supervise and monitor the performance of assigned home health care teams to ensure high-quality care and compliance with regulatory requirements.
- Develop and implement individualized care plans that address the unique needs of patients, including those with complex medical conditions.
- Collaborate with interdisciplinary teams, including physicians, therapists, and social workers, to ensure seamless care coordination and communication.
- Conduct regular quality assurance and performance improvement activities to identify areas for improvement and implement changes to enhance patient outcomes.
- Provide education and training to home health care teams on best practices, regulatory requirements, and new technologies.
- Two years of experience in a licensed and certified home health agency.
- Competent in Federal (Medicare) and Local standards, including Conditions of Participation and Local Coverage Determinations.
- Supervisory experience preferred.
- Current/Active Registered Nurse (RN) License within practice state required.
- Reliable transportation and proof of valid automobile liability insurance.
