We are seeking a dedicated and compassionate Medical Case Manager to join our AmeriHealth Caritas DC Clinical Support team in Washington, DC.

About the Role:This Medical Case Manager position requires engagement with members, caregivers, and healthcare providers to assess, plan, and establish individualized goals. The ideal candidate will work collaboratively to coordinate care across various settings, ensuring seamless transitions and optimized outcomes for patients.
- Conduct thorough assessments to determine care coordination needs, taking into account physical health, psycho-social factors, environmental considerations, and social determinants of health.
- IDentify problems or barriers and implement targeted interventions to support member well-being.
- Cultivate strong relationships with healthcare teams, community resources, and other stakeholders to facilitate effective care management.
To be successful in this role, you should have:
- A valid state RN license or current LICSW licensure.
- A bachelor's degree in a health-related field (or higher) and relevant experience working with Medicaid populations.
- Certification as a case manager is preferred but not required.
- At least three years of nursing or social work experience, preferably in a hospital or community health setting.
- Ability to work effectively with adult and pediatric populations presenting complex medical and behavioral health challenges.
