Job Purpose:
Enhance the health outcomes of the communities served by providing support to patients with intricate chronic conditions as follows:
Key Responsibilities:
- Evaluates available registries and reports to pinpoint patients with complex chronic conditions requiring additional assistance, facilitating the implementation of suitable care plans.
- Conducts comprehensive patient chart reviews and interviews to uncover clinical and social determinants of health challenges.
- Collaborates with patients, CHAS Health care teams, and external organizations to ensure coordinated care.
- Assists patient care teams in developing care plans and aids in their execution to enhance patient engagement and adherence to prescribed therapies. Provides patient education as necessary.
- Works alongside case management and other resources to address social determinants of health needs and challenges.
- Facilitates effective transitions of care for patients leaving hospital settings.
- Helps track and present outcomes for designated populations to leadership.
- Performs additional duties as assigned, supporting the CHAS Health Mission and Core Values.
Education: Bachelor's degree in nursing from an accredited institution or equivalent experience required. Registered Nurse (RN) license in the State of Washington required upon hire and eligibility to obtain RN license in Idaho. Experience in community health settings preferred. Familiarity with Chronic Disease Management preferred. Experience with electronic health records preferred.
Skills: Strong customer service, oral and written communication skills required. Compassion for patients is essential. Ability to work effectively in a team environment is necessary. Must be capable of multi-tasking in a fast-paced setting, with a proactive approach to anticipating future needs. Commitment to fostering a safe, respectful, equitable, and inclusive workplace is required. Proficient computer skills are necessary. CPR/AED certification for Health Care Providers is required. Ability to mentor students as needed. Valid driver's license and insurance are required.
Physical Requirements:
While performing the duties of this position, the employee is required to stand, sit, and be mobile one-third to two-thirds of the time. They must use hands to finger, handle, or feel over two-thirds of the time, as well as reach with hands and arms. Climbing or balancing occurs less than one-third of the time; while stooping, kneeling, or crouching occurs between one-third and two-thirds of the day. Communicating by talking/hearing occurs over two-thirds of the day. The amount of time spent lifting or exerting force is about 50% for up to 10 pounds and less than one-third of the time for up to 25 pounds. Rarely is there a need to lift more than 26 pounds.
