About the Role:
As a Patient Care Specialist, you will play a vital role in ensuring the smooth transition of patients from hospital to home. You will work closely with patients, families, and healthcare teams to provide comprehensive assessments, develop personalized care plans, and coordinate care services. Your expertise and compassion will make a significant impact on the lives of our patients and their loved ones.
Responsibilities:
- Conduct comprehensive assessments of patients' physical, emotional, and social needs
- Develop and implement personalized care plans to meet patients' unique needs
- Coordinate care services with healthcare teams, families, and community resources
- Provide education and support to patients and families on self-care, disease management, and health promotion
- Collaborate with healthcare teams to ensure seamless transitions of care
- Document patient information accurately and efficiently
Requirements:
- Certified Nurse Practitioner (NP) or Physician Assistant (PA) through a national board
- Active and unrestricted license in the state of practice
- Current active DEA licensure/prescriptive authority or ability to obtain post-hire, per state regulations
- Access to reliable transportation for travel to client and/or patient sites
- Ability to lift a 30-pound bag in and out of car and navigate stairs and various dwelling conditions
- Availability to work 24 hours per month, with expectations of 16 hours/month during off-hours
Preferred Qualifications:
- 1+ years of hands-on post-grad experience in Long Term Care
- Understanding of Geriatrics and Chronic Illness
- Understanding of Advanced Illness and end-of-life discussions
- Proficient computer skills, including documentation of medical information with written and electronic medical records
- Ability to develop and maintain positive customer relationships
- Adaptability to change
