Job Overview:


The Clinical Care Coordinator employs specialized knowledge to conduct comprehensive psychosocial evaluations, formulate and execute tailored care strategies in collaboration with the healthcare team, assess urgent situations to provide essential resources or referrals for patients, and deliver individual and family therapy as necessary.

This role involves working closely with patients, their significant others, families, healthcare professionals, insurance representatives, community organizations, and all other relevant stakeholders.

The Clinical Care Coordinator (CCC) applies clinical skills to carry out psychosocial assessments and create/implement a treatment plan that ensures exceptional care for patients while being mindful of costs and available resources.

This position collaborates with patients, their families/significant others, healthcare providers, insurers, community resources, and other involved parties to identify patient needs, facilitate services, coordinate care and transition plans, and advocate for patients.

The CCC performs these responsibilities under the direct supervision of a Licensed Clinical Social Worker (LCSW).

Position Details

Compensation Range Clinical:
$34.06-$52.55 Non-Exempt

Compensation Range Certified:
$30.94-$47.77 Non-Exempt

$43.42 Non-Exempt

Work Schedule:
PRN 24 hrs/week, three 8-hour shifts per week, varying days, may include weekends.

Key Responsibilities

Evaluates the patient's medical and mental health history along with current conditions to ascertain treatment needs.
Organizes and prioritizes daily tasks by assessing the needs of new, current, and discharging patients within assigned areas.
Diagnoses and addresses various mental, emotional, and behavioral disorders.
Conducts social work consultations for patients and caregivers, performing psychosocial assessments and providing necessary interventions including crisis support and bereavement assistance.

Develops individualized care plans with input from patients, their families, physicians, and other healthcare team members.

Adjusts care plans to reflect changes in patient conditions.
Refers patients to additional services to enhance their well-being, which may include community resources, treatment programs, support groups, psychiatrists, or other mental health professionals.

Educates, communicates, and collaborates with multiple patient care teams and external agencies to ensure quality care, timely discharges, and to identify and resolve any delays or issues.

Credentialing through the medical staff office and peer review processes may be required if working in behavioral health settings.
May provide support and debriefing for staff members following adverse events in conjunction with the Employee Assistance Program.

Acts as an advocate for patients and families to ensure effective communication with the multidisciplinary care team, external agencies, and other necessary resources.

Collaborates with the multidisciplinary team to create discharge plans that provide continued care, services, and referrals to ensure adherence to continuity of care standards.

Documentation and Compliance

Completes timely and accurate documentation in the medical record, adhering to departmental standards to facilitate communication among team members.

Documentation is maintained in compliance with all clinical guidelines and
Complies with federal, state, local, and clinical program/facility standards.
Operates within the scope of the Mental Health Practice Act.
Identifies patients at risk for potential avoidable delays, extended stays, readmissions, or complex discharge needs, coordinating with care providers and leaders to achieve optimal solutions.

Qualifications

Master's Degree in Social Work from an accredited institution, with verification of degree required.
Current State Licensure as a Social Service Worker (SSW), Certified Social Worker (CSW), or Licensed Clinical Social Worker (LCSW)
Proficient computer skills (email, word processing, and spreadsheets).

Preferred Qualifications

Experience in a healthcare environment.

Physical Requirements:
Ongoing need for the employee to see and read information, labels, assess patient needs, operate monitors, and identify equipment and supplies.

Frequent interactions with patient care providers, patients, and visitors require verbal communication and the ability to hear and understand spoken information, alarms, needs, and issues quickly and accurately, especially during emergencies.

Manual dexterity is necessary for manipulating complex and delicate equipment with precision. This includes frequent computer use and typing for documenting patient care, accessing information, and preparing medications.
Must be able to walk and assist with transporting patients and obtaining and distributing supplies and equipment, which may involve pushing/pulling gurneys and portable equipment, including heavy items. Often required to navigate crowded and busy environments.
May be expected to stand in a stationary position for extended periods.
