Home Health LPN Per Diem Job Description

**Job Summary:**

As a Home Health LPN Per Diem, you will provide professional nursing services under the direction of a RN in compliance with healthcare standards and regulations. You will provide skilled nursing interventions and holistic care, confer with the RN Supervisor, and accept verbal orders from physicians. You will maintain contact with patients, physicians, and clinical manager(s), and maintain the highest standards of professional conduct and adherence to professional standards. You will prepare, submit, and maintain documentation as required.

**Required Experience/Skills:**

* Graduate of an accredited LPN program
* Current nursing license in the practicing state
* Valid drivers license, auto insurance, and reliable transportation
* Current CPR certification
* Two years experience as an LPN/LVN in a clinical setting
