Job Title: LPN - Full Time Evenings at The Glen

We are seeking a skilled and compassionate Licensed Practical Nurse (LPN) to join our team at The Glen at Hiland Meadows in Queensbury, NY. As a LPN, you will play a vital role in providing high-quality care to our residents, working collaboratively with our interdisciplinary team to ensure the best possible outcomes.

Responsibilities:
  • Provide direct patient care to residents, including administering medications, performing vital signs, and providing wound care.
  • Oversee and mentor a team of resident assistants, ensuring they are equipped to provide excellent care to our residents.
  • Develop and implement individualized care plans, taking into account the unique needs and preferences of each resident.
  • Collaborate with the nursing director and other healthcare professionals to ensure seamless care transitions and effective communication.
  • Participate in ongoing education and training to stay up-to-date on the latest best practices and technologies in long-term care.
Requirements:
  • Valid NYS LPN license
  • 1+ year of experience working in a direct patient care role, handling med passes
  • 1+ year of experience as a LPN, with a willingness to help train the right candidate to further their skills
  • Understanding of assisted living regulations
  • Strong leadership and mentorship skills, with the ability to motivate and inspire a team
  • Excellent communication and interpersonal skills, with the ability to work effectively with residents, families, and healthcare professionals
