About the Role:The RN, Case Manager will provide and coordinate nursing care to hospice patients and their families/caregivers. Working collaboratively with the interdisciplinary team, the RN, Case Manager will plan and manage the nursing component of the hospice caseload.
Responsibilities:- Make initial and ongoing nursing assessments, judgments, and treatment decisions based on patient and family/caregiver needs and wishes.
- Supervise LPNs, CNAs, and Patient Care Volunteers.
- Develop and implement individualized care plans that meet the unique needs of each patient.
- Collaborate with the interdisciplinary team to ensure comprehensive care and support for patients and their families.
- Provide education and support to patients and their families on end-of-life care and related topics.
- At least one year of staff nurse experience in a comparable setting/unit.
- Hospice and/or palliative care experience is strongly preferred.
- Current licensure from the governing body for practice for the District of Columbia, State of Maryland, and/or the Commonwealth of Virginia, based on the state/jurisdiction of practice.
- Healthcare Provider BLS (CPR) certification is required to practice in the State of Maryland and the District of Columbia.
- BSN degree.
- CHPN certification.
