Social Work Specialist

The Social Worker provides social work services to primary care patients in participating Vermont Blueprint for Health Program and participates as an active member of the Community Health Team.
Key Responsibilities- Provide social work services to primary care patients
- Participate as an active member of the Community Health Team
- Develop and implement social work plans to meet patient needs
- Collaborate with healthcare providers to ensure comprehensive patient care
- Provide education and support to patients and families
- Two (2) years social work experience in a community, home health, or outpatient practice setting
- Knowledge of a variety of local, state, and federal resources
- Expertise in advanced care planning strongly preferred
- Experience with interventions with adults, families, and children strongly preferred
- Valid license in social work, vocational rehabilitation, substance abuse counseling, or related field required
- Valid Driver's License and Auto Insurance required
8:00AM to 5:00PM
