About the Role

We are seeking a skilled and detail-oriented Referral Coordinator Specialist to join our team at Heartland Community Health Center.

This role plays a vital part in ensuring seamless patient care, coordinating referrals, and communicating with healthcare providers. The successful candidate will have strong organizational skills, excellent communication abilities, and a passion for delivering exceptional patient experiences.

Key Responsibilities:
  • Coordinate all patient referrals and radiology orders, tracking progress and reporting on results
  • Schedule appointments and communicate with patients and healthcare providers as needed
  • Notify patients of upcoming appointments, provide instructions, and ensure smooth transitions
  • Enter referrals into the electronic health record system, track reports, and maintain accurate records
Requirements:
  • Preferred medical office experience, knowledge of medical terminology, and HIPAA compliance
  • Ability to work under pressure, multitask, and maintain a calm demeanor in fast-paced environments
  • Strong communication and interpersonal skills, with a focus on empathy and patient-centered care
  • Basic computer skills, including word processing, office procedures, and business machines

We offer a competitive salary range of $43,000 - $58,000 per year, based on experience, and a comprehensive benefits package.
