Care Management Coordinator

As a Shelter Case Manager, you will play a critical role in providing support and assistance to individuals and families experiencing homelessness.
You will be responsible for managing a caseload of clients, assessing their needs, and developing and implementing individualized service plans to help them achieve their goals.
Your work will directly contribute to our mission of ending homelessness and improving the lives of those we serve.
Key qualifications include:
- Bachelor's degree in social work, psychology, or a related field
- Experience working with individuals experiencing homelessness or in a related field
- Master's degree in social work, psychology, or a related field
- Knowledge of local resources and services for individuals experiencing homelessness
- Bilingual a plus
As a Shelter Case Manager, you will use your strong communication and interpersonal skills to build relationships with clients and community partners.
You will also use your knowledge of local resources and services to connect clients with the support they need.
Additionally, your ability to work independently and as part of a team will be critical in achieving our mission of ending homelessness and improving the lives of those we serve.
