Care Coordinator

We are seeking a skilled Care Coordinator to join our team at Sutter Health. As a key member of our care management team, you will play a vital role in ensuring the timely progression and transition of patients to the appropriate level of care. This position requires strong communication and collaboration skills, as well as the ability to work effectively with patients, families, physicians, and other healthcare professionals.
Key Responsibilities:
Develop and implement individualized care plans to meet the unique needs of each patient
Collaborate with patients, families, and healthcare teams to ensure seamless transitions and optimal health outcomes
Utilize strong communication and negotiation skills to facilitate care coordination and resolve complex clinical and social situations
Work closely with physicians, nurses, and other healthcare professionals to ensure timely and effective care transitions
Maintain accurate and up-to-date records of patient interactions and care plans
Requirements:
Graduate of an accredited school of nursing
Current RN licensure in California
2 years of recent relevant experience in care coordination or a related field
Strong knowledge of healthcare delivery systems, case management, and utilization review
Excellent communication and interpersonal skills
Ability to work independently and as part of a team
