Job Overview

We are seeking a highly skilled Nurse Navigator to join our team at MedStar Health. As a key member of our healthcare organization, you will play a vital role in providing exceptional patient care and experience.

About the Role

The successful candidate will be responsible for coordinating patient care across the continuum, working closely with interdisciplinary teams to develop and manage individualized plans of care. This includes obtaining baseline information, scheduling ancillary testing, and presenting necessary history and diagnostic studies to ensure seamless care transitions.

Main Responsibilities

  • Assessment: Gather comprehensive information to inform plan development and ensure accurate representation of patient needs.
  • Planning: Collaborate with healthcare teams to create and manage individualized care plans, prioritizing patient goals and preferences.
  • Evaluation: Monitor patient progress, adjusting care plans as needed to optimize outcomes and satisfaction.
  • Patient Education: Provide clear, concise information to patients and families, empowering them to make informed decisions about their care.
  • Quality Improvement: Participate in quality initiatives, contributing to the development of internal clinical guidelines and pathways to enhance patient care.

Requirements

  • Education: Bachelor's degree in Nursing (BSN) required; Master's degree in Nursing preferred.
  • Experience: 3-4 years of progressively more responsible clinical nursing experience in specialty areas required; Case Management or Oncology experience may be preferred based on location.
  • Licenses and Certifications: RN - Registered Nurse - State Licensure and/or Compact State Licensure in the State of Maryland or District of Columbia Upon Hire required; AOCN - Advanced Oncology Certified Nurse based on location Upon Hire preferred; CPR - Cardiac Pulmonary Resuscitation (includes BLS and NRP) for healthcare providers from either the American Heart Association (AHA) or American Red Cross within 90 Days required.
