We are seeking a highly skilled Registered Nurse to join our interdisciplinary team as a Hospice Care Specialist. As a key member of our team, you will provide comprehensive care to patients and families facing terminal illness and bereavement.
Key Responsibilities- Coordinate care and ensure continuous assessment of patients' and families' needs
- Implement the interdisciplinary plan of care and provide supervision of nursing tasks delegated to Hospice Aides, Licensed Practical/Vocational Nurses, and Volunteers
- Complete initial and ongoing comprehensive assessments, including physical, psychosocial, spiritual, and bereavement evaluations
- Identify and utilize resources, referrals, and services to support patients and families
- Provide education and training to patients, families, caregivers, and referral sources on palliative care and pain management
- Participate in patient care meetings, weekly team meetings, and bereavement support
- Provide effective communication and shift reports through written, verbal, and body language
- Participate in the on-call rotation and provide shifts during non-business hours as needed
- Current CA RN license
- BSN or LVN with over a year of experience
- Ability to work in a fast-paced environment and prioritize multiple tasks
