We are seeking a dedicated and compassionate Obstetric Medicine Doula to join our team at Yale New Haven Health. As a Patient Care Navigator, you will play a vital role in providing exceptional care to our patients attending the Collaborative Obstetric Medicine clinic and Postpartum Heart Care Program.
Key Responsibilities- Provide education and support to patients about the Postpartum Healthcare Program
- Follow up with patients after ED visits and check in on those with mental health needs
- Remind patients to complete labs and review lifestyle plans
- Perform screenings for social determinants of health and connect patients with resources
To be successful in this role, you will need a High School Diploma or GED and completion of approved DONA International doula training. Experience working with birthing families and prenatal and postnatal care is also required.
This is a grant-funded position, and we are excited to offer a unique opportunity for a dedicated professional to make a meaningful impact in the lives of our patients.
