Patient Care Navigator

We are seeking a dedicated and compassionate Obstetric Medicine Doula to join our team at Yale New Haven Health. As a Patient Care Navigator, you will play a vital role in providing exceptional care to our patients attending the Collaborative Obstetric Medicine clinic and Postpartum Heart Care Program.
Key Responsibilities- Provide education and support to patients about the Postpartum Healthcare Program
- Follow up with patients after ED visits and check in on those with mental health needs
- Remind patients to complete labs and review lifestyle plans
- Perform screenings for social determinants of health and connect patients with resources
To be successful in this role, you will need:
- A High School Diploma or GED
- Completion of approved DONA International doula training
- Experience working with birthing families and prenatal and postnatal care
