About the Role

We are seeking a highly skilled and compassionate Nurse Practitioner to join our team at Oak Street Health Inc. As a key member of our clinical care team, you will play a vital role in delivering high-quality patient care and contributing to the success of our organization.

Key Responsibilities
  • Provide Comprehensive Patient Care: Utilize your expertise to assess, diagnose, and treat patients with a range of medical conditions, ensuring they receive the highest level of care and attention.
  • Collaborate with Interdisciplinary Teams: Work closely with our healthcare professionals, including physicians, nurses, and other support staff, to develop and implement effective care plans that meet the unique needs of each patient.
  • Conduct Thorough Assessments: Perform comprehensive assessments of patients to identify their medical needs, develop treatment plans, and provide ongoing care and support.
  • Manage Patient Medications: Ensure accurate and timely management of patient medications, including prescription refills, prior authorizations, and medication education.
  • Provide Education and Support: Offer patients and their families comprehensive education and support to help them manage their health conditions, make informed decisions, and achieve optimal health outcomes.
  • Participate in Quality Improvement Initiatives: Contribute to ongoing quality improvement efforts, including data analysis, process improvement, and best practice implementation, to ensure our patients receive the highest quality care.
Requirements
  • Active RN Licensure: Possess an active and unencumbered RN license in good standing with the applicable state.
  • Electronic Medical Record (EMR) Experience: Have experience working with electronic medical records and a strong understanding of EMR systems.
  • Strong Communication and Assessment Skills: Demonstrate excellent communication and assessment skills, with the ability to work effectively with patients, families, and healthcare teams.
  • US Work Authorization: Be authorized to work in the United States.
Preferred Qualifications
  • Previous Experience in a Clinic Setting: Possess experience working in a clinic or healthcare setting, with a strong understanding of clinical operations and patient care.
  • Fluency in Spanish, Polish, Russian, or Other Languages: Be fluent in Spanish, Polish, Russian, or other languages spoken by people in the communities we serve.
