- Develop and Implement Care Plans: Collaborate with patients, families, and healthcare teams to create personalized care plans that address their unique needs and goals.
- Conduct Health Education and Promotion: Educate patients and their families on healthy lifestyle choices, disease management, and wellness strategies to promote optimal health outcomes.
- Collaborate with Healthcare Teams: Work closely with physicians, nurses, and other healthcare professionals to ensure seamless care coordination and communication.
- Manage Patient Records and Data: Maintain accurate and up-to-date patient records, including medical histories, test results, and treatment plans.
- Participate in Quality Improvement Initiatives: Contribute to quality improvement efforts by identifying areas for improvement and implementing evidence-based practices to enhance patient care and outcomes.
- Active RN License: Possess an active and unencumbered RN license in the state where you will be practicing.
- Experience in Patient Care: Have a minimum of 2 years of experience in patient care, preferably in a clinical or healthcare setting.
- Strong Communication Skills: Demonstrate excellent communication and interpersonal skills, with the ability to work effectively with patients, families, and healthcare teams.
- Electronic Medical Record (EMR) Experience: Have experience using EMRs and other healthcare technology systems.
- Ability to Work Independently: Possess the ability to work independently and as part of a team, with a strong focus on patient-centered care.
- Experience in a Clinic Setting: Have experience working in a clinic or healthcare setting, preferably in a role that involves patient care and coordination.
- Fluency in Spanish or Other Languages: Possess fluency in Spanish or other languages spoken by patients in our communities, as needed.
- Leadership and Management Experience: Have experience in leadership or management roles, preferably in a healthcare setting.
