Seasonal Registered Nurse Case Manager

We are seeking a highly skilled Seasonal Registered Nurse Case Manager to join our team at St. David's South Austin Medical Center. As a key member of our rehabilitation team, you will play a vital role in ensuring that our patients receive the highest quality care and achieve optimal outcomes.
Responsibilities- Provide program orientation to patients, families, and caregivers, including an overview of the case manager's role, rehabilitation philosophy, and continued stay and discharge criteria.
- Act as the coordinator of patient, family, and caregiver education, promoting participation in team discussions related to plans, goals, and status.
- Ensure the implementation of the patient's treatment plan, supporting the patient's strengths, abilities, needs, and preferences.
- Document the findings of the Discharge Planning Evaluation (DPE) and psychosocial assessments, communicating social, financial, or discharge needs and preferences of the patient, family, and caregiver.
- Promote consistent, positive patient interactions that advance the agenda of unparalleled patient service.
- 2+ years' experience in case management in a hospital setting.
- InterQual experience preferred.
- Associate Degree in Nursing or Nursing Diploma.
- Bachelor's Degree in Nursing preferred.
- Currently licensed as a Registered Nurse in the state(s) of practice according to law and regulation.
- Certification in Case Management preferred.
St. David's South Austin Medical Center is a 368-bed acute care facility offering a range of complex specialties and sub-specialties. Our services include a nationally accredited oncology program with the area's only adult Transplant and Cellular Therapy Program, advanced trauma care with a Level II trauma center, comprehensive cardiac, stroke, and orthopedic programs, full-service maternity and newborn care, and two full-service emergency centers in the communities of Bee Cave and Bastrop.
We offer advanced robotic services for orthopedics, open heart and lung surgery, complex GI surgery, surgical oncology, general surgery, colorectal surgery, and urology. It was one of the first hospitals in Central Texas to combine the use of CT imaging and robotics to detect early stage lung cancer.
