Overview

  • Position Overview
  • Accountable for the enrollment of participants into the healthcare program. Evaluates the emotional, social, and environmental requirements of participants. As necessary, delivers case management services including information and referral, psychosocial support, and advocacy. Collaborates with healthcare, mental health, and social service providers alongside the participant to enhance the enrollee's health and their capacity to engage in the program.
  • Essential Qualifications, Skills, and Abilities
  • Education: A Bachelor's degree is preferred or equivalent relevant experience in a healthcare setting.
  • Licenses/Certifications: May require travel to various locations. A valid driver's license and reliable transportation are necessary.
  • Experience: Approximately two years of experience in a community health environment is required to solidify and demonstrate knowledge of community resources and the ability to assist individuals in making informed health decisions. Capability to ensure that various disciplines or community resources collaborate effectively to enhance the health and well-being of the enrollee.
  • A considerable level of analytical skills and sound judgment in evaluating and planning for the needs of participants.
  • Comfort and proficiency in developing and delivering presentations to groups of healthcare and service providers is advantageous.
  • Ability to utilize software systems that support program functions, including database management and statistical report generation.
  • Strong customer service, teamwork, resource knowledge, and advocacy skills are crucial.
