Job Summary

We are seeking a highly skilled LPN Medication Care Manager to join our team at Sunrise Senior Living. As a Medication Care Manager, you will be responsible for providing high-quality care and services to our residents by administering medication and treatments in a safe and organized manner.

Key Responsibilities:

  • Administer medications and treatments to residents in accordance with their individualized care plans.
  • Document and maintain accurate records of medication administration, including dosages, times, and any adverse reactions.
  • Conduct regular audits of medication carts and treatment areas to ensure compliance with safety protocols.
  • Collaborate with the Resident Care Director and Wellness Nurse to ensure accurate and timely medication administration.
  • Participate in the development and implementation of Individualized Service Plans (ISPs) and Individualized Care Plans (ICPs).

Requirements:

  • Current LPN licensure in the state of New York.
  • Previous experience working with seniors and administering medications.
  • Ability to work effectively in a fast-paced environment and prioritize tasks accordingly.
  • Strong communication and interpersonal skills, with the ability to work collaboratively with residents, families, and healthcare professionals.
  • Ability to maintain confidentiality and handle sensitive information with discretion.
