Job Description

We are seeking a skilled Licensed Practical Nurse (LPN) to fill the role of Care Coordinator. As a key member of our interdisciplinary healthcare team, you will be responsible for developing and implementing care plans, managing patient care transitions, and coordinating care plans.

Responsibilities:

  • Collaborate with healthcare providers, patients, and families to develop individualized care plans.
  • Serve as a liaison between patients, families, and healthcare providers, advocating for patient needs and preferences.
  • Provide education to patients and their families on disease management, medication adherence, and healthy lifestyle choices.
  • Track patient progress, identify potential issues, and coordinate follow-up care to ensure continuity and quality of care.
  • Maintain accurate and up-to-date patient records, care plans, and reports in compliance with healthcare regulations and standards.
  • Assist with navigating the healthcare system, including arranging appointments, referrals, and transportation as needed.
  • Participate in quality improvement initiatives to enhance patient care and outcomes.

Requirements:

  • Current LPN (Licensed Practical Nurse) license in State of practice.
  • Minimum of 6 Months Long Term Care experience.
  • Geriatric behavioral health experience preferred.
  • Strong communication, interpersonal, and organizational skills.
  • Ability to work effectively in a team-oriented environment.
  • Proficiency in MS Suite, electronic medical records (EMR) and other relevant healthcare software.
  • Compassionate, patient-centered approach to care.
  • Basic Life Support (BLS) certification required.
