Job Description

We are seeking an experienced RN to join our team as a Hospice Care Nurse. As a member of our interdisciplinary team, you will provide skilled nursing care and coordination to patients admitted to our hospice house inpatient services.

A key responsibility of this role is to perform initial evaluations of patients and apply admission criteria. You will also provide skilled nursing care and education to patients and their caregivers, developing, coordinating, modifying, and managing individualized plans of care as needed.

To succeed in this role, you must have a strong foundation in nursing principles and practices. A minimum of 1 year of experience in home health or hospice/end of life nursing is required, along with a current Registered Nurse license in the state of practice and Basic Life Support certification.

As a Hospice Care Nurse, you will work closely with other members of the healthcare team to ensure seamless care transitions and effective patient outcomes. Your excellent communication and interpersonal skills will enable you to build strong relationships with patients, families, and colleagues.

Requirements

* 1+ year of experience in home health or hospice/end of life nursing
* Current Registered Nurse license in the state of practice
* Basic Life Support certification

Compensation

We offer a highly competitive salary range of $48,220 - $75,330 per year. Additionally, PeaceHealth provides a comprehensive benefits package, including medical, dental, and vision coverage, as well as a 403(b) retirement plan and paid time off.
