Job Summary
We are seeking a qualified End-of-Life Care Specialist to join our team. The ideal candidate will have experience in hospice care and be able to provide top-notch patient care in the comfort of their own homes.
Responsibilities:
- Provide personalized care to patients, including bathing, grooming, and other activities of daily living
- Take accurate vital signs and document any changes or concerns
- Assist patients with mobility and ambulation as needed
- Support patients and families through difficult times, offering emotional support and guidance
- Collaborate with licensed staff to ensure seamless care coordination
Requirements:
- Current CPR certification
- Valid driver's license and reliable transportation
- Excellent communication and interpersonal skills
- Able to work independently and as part of a team
- Strong understanding of hospice care principles and regulations
