Nurse Care Coordinator for Hospice Patients

This position involves providing skilled nursing care and coordination/management of patients admitted to hospice house inpatient services using considerable judgment and skill. The nurse is responsible for performing initial evaluations, developing and managing patient care plans, and coordinating with the interdisciplinary team to ensure smooth care flow.
Key Responsibilities:
- Perform initial evaluations of patients to determine suitability for home health/hospice services.
- Provide skilled nursing care and education to patients and caregivers according to the hospice plan of treatment.
- Develop, coordinate, modify, and manage written individualized plans of care as ordered by the primary physician.
- Collaborate with the interdisciplinary team to ensure appropriate and timely hospice outcomes.
- Perform discharge planning and evaluate interventions and outcomes of patient care by reviewing long and short-term goals established in the plan of care.
Requirements:
Education:
* Accredited School of Nursing Required: Nursing
* Bachelor's Degree Preferred: Bachelor of Science in Nursing
Experience:
* Minimum of 1 year Required: Home health or hospice/end of life nursing experience
* Preferred: Hospice experience
Credentials:
* Registered Nurse in state of practice Required Upon Hire
* Basic Life Support Required Upon Hire
* Food Handlers Card - Washington Required Upon Hire
Salary: $58,335 - $88,497 annually, based on location and experience.
