Nursing Care Coordinator for Hospice Patients

Job Summary:
Precise and skilled nursing care coordination is required to manage patients admitted to hospice house inpatient services. This role involves utilizing professional healthcare processes to make informed patient care decisions, with a focus on assessment, planning, intervention, and evaluation.

Responsibilities:
* Perform initial evaluations of patients and apply admission criteria to determine suitability for home health or hospice services.
* Provide skilled nursing care and education to patients and caregivers according to the hospice plan of treatment.
* Conduct physical assessments, develop, coordinate, modify, and manage individualized plans of care as ordered by primary physicians to meet patient goals and outcomes.
* Develop and manage Home Health Aide care plans based on patient evolving personal care needs.
* Coordinate care, collaborate, and communicate with interdisciplinary teams to ensure smooth care continuum flow.
* Plan and implement discharge planning towards patient-specific goals and outcomes, evaluating interventions and outcomes through reviewing long and short-term goals established in the plan of care.
* Maintain accurate, complete, and timely clinical documentation in compliance with regulatory, quality, and agency guidelines.
* Assist with bereavement services for patient deaths and perform other duties as assigned.

Qualifications:
* Education: Accredited School of Nursing; Bachelor's Degree in Nursing preferred.
* Experience: Minimum 1 year in home health or hospice/end-of-life nursing, with demonstrated knowledge of age-specific growth and development principles and life stages to meet patient needs.
* Credentials: Registered Nurse in state of practice; Basic Life Support; Food Handlers Card - Washington.
* Skills: Basic computer skills, including MS Office applications; well-organized, flexible, and able to function independently; cooperative working skills, leadership skills, and interpersonal abilities to work effectively with diverse patients, families, and staff.

Working Conditions:
Lifting up to 35 lbs without assistive equipment; fine motor skills for grasping medical equipment; pushing/pulling up to 45 lbs force; performing hands-on CPR using 100-125 lbs force; frequent sitting and moving around areas; bending/stooping/squatting/reaching/kneeling frequently; lifting up to 20 lbs occasionally and 5 lbs frequently.

Mental/Visual:
Vision and hearing within normal limits; ability to communicate and exchange accurate information.
