We are seeking a skilled and compassionate Nurse Coordinator to join our team at Carespring Health Care Management in Loveland, OH. This is a full-time position offering a competitive salary of $44 per hour.

This role will focus on overseeing the care planning process for new and existing patients, ensuring that all their needs are addressed and documented on the Minimum Data Set (MDS).

Key Responsibilities:

  • Coordinate with other healthcare professionals to complete initial assessments for new admissions
  • Manage patient admissions scheduling, MDS assessment, and care planning
  • Review and submit required documents weekly or as directed by the Director of Nursing
  • Analyze validation reports to ensure error-free submissions
  • Collaborate with interdisciplinary teams to improve resident outcomes

Requirements:

  • A current LPN or RN license in the state of Ohio
  • Flexible work schedule and ability to work independently
  • Knowledge of the MDS process and willingness to continually learn
  • Understanding of PDPM and RUG systems
