About the Job: We are seeking a highly motivated and experienced Care Management Clinical Excel Coord to join our team at Parkview Health. As a key member of our Care Management team, you will play a critical role in planning, coordinating, and facilitating education initiatives to support our mission and vision.

Key Responsibilities:
  1. Develop and implement education plans to enhance coworker knowledge and skills.
  2. Collaborate with coworkers to identify areas for improvement and develop strategies to address them.
  3. Conduct routine audits to ensure accurate and compliant documentation.
  4. Analyze data to identify potential quality issues and report findings to the management team.
  5. Communicate changes and updates to coworkers and provide education as needed.
  6. Recommend performance improvement opportunities and collaborate with the management team to implement changes.

Qualifications:
  • Bachelor's degree in Nursing from an accredited school.
  • Masters degree in Healthcare related field or MSN preferred.
  • Current Indiana state license as an RN.
  • 3-5 years of nursing experience.
  • Leadership experience preferred.
  • 3-5 years care management experience preferably in Home Health, Ambulatory Care, Community Public Health, Case Management, and Coordinating Care across multiple settings and with multiple providers.

Required Skills:
  • Able to work independently and as part of a team.
  • Excellent communication and interpersonal skills.
  • Ability to analyze data and make recommendations.
  • Strong problem-solving skills and ability to adapt to changing situations.

Salary Range: $85,000 - $110,000 per year
