Salary: $130,000 - $160,000 per year

Job Description: As a Community Healthcare Specialist, you will be responsible for managing and providing pharmaceutical care in our independent community pharmacy setting. This role requires a strong understanding of medication therapy and the ability to develop personalized medication action plans.

Key Responsibilities:

  • Deliver patient-centered care by dispensing medications accurately and efficiently.
  • Conduct thorough medication reviews to identify and address medication-related issues.
  • Provide patients with education and guidance on medication use, potential side effects, and health maintenance.
  • Collaborate with healthcare providers to optimize medication therapy and improve patient outcomes.
  • Manage pharmacy operations, including inventory and procurement, to ensure seamless delivery of services.
  • Participate in quality assurance programs and continuous improvement initiatives to enhance pharmacy services.

Requirements:

  • Bachelor's of Pharmacy or PharmD degree from an accredited pharmacy program.
  • Experience in community or independent pharmacy.
  • State pharmacy license in good standing.
  • Proficiency in utilizing pharmacy management software and EHR systems.
