Advanced Practice Provider Surgical Specialist

The Advanced Practice Provider Surgical role at Prevea Health is a dynamic opportunity for a skilled professional to deliver high-quality patient care in a collaborative environment.
Key Responsibilities- Perform comprehensive health assessments for acute and chronic patients, identifying and addressing their unique needs and concerns.
- Assess and manage care for acute, minor, episodic illnesses and injuries, providing timely and effective interventions.
- Interview and advise patients and families on health and illness prevention strategies, empowering them to take control of their well-being.
- Conduct follow-up exams to monitor patient progress and adjust treatment plans as needed.
- Collaborate with patients to determine their physiologic, emotional, and environmental needs and priorities, developing personalized care plans.
- Provide case management services to assist clients and families with arranging for or referring them to needed services.
- Manage patient health concerns through telephone triage and consultation, ensuring timely and effective care.
- Order diagnostic and laboratory tests, prescribe pharmacological agents, and provide non-pharmacological treatment modalities as needed.
- Maintain and review patient records, charts, and other pertinent information, ensuring accurate and up-to-date documentation.
- Perform patient callbacks, report diagnostic findings, and provide consultation to ensure seamless care.
- Participate in call coverage when required, collaborating with physicians and other healthcare professionals to ensure continuity of care.
- Attend required department meetings and participate in committees as requested, contributing to ongoing quality improvement activities.
- Perform other clinical duties as assigned by supervisory physicians in accordance with experience and scope of practice.
- Graduate of a master's level nurse practitioner program or an accredited physician assistant program.
- Experience requirements may vary.
- Willingness to learn new roles within scope of practice.
- Ability to develop and maintain effective working relationships.
- Organizational skills to ensure timely patient appointments and follow-up.
- Presents a courteous and service-oriented image through positive and helpful interaction with patients, families, physicians, and co-workers.
- Participates in APP activities throughout Prevea and the community.
- Participates in community activities.
- Licenses and certifications as required for a Nurse Practitioner or Physician Assistant.
