Job Title: APRN - Advanced Practice Registered Nurse - Palliative Care Specialist

Trinity Health is seeking an experienced APRN - Advanced Practice Registered Nurse - Palliative Care Specialist to join our team. As a key member of our palliative care team, you will provide high-quality patient care and support to patients and families in need of palliative care services.

Job Summary:

The APRN - Advanced Practice Registered Nurse - Palliative Care Specialist will work collaboratively with our interdisciplinary team to provide comprehensive care to patients with serious illnesses. This includes assessing patients' physical, emotional, and spiritual needs, developing and implementing individualized care plans, and providing education and support to patients and families.

Responsibilities:
  • Assess patients' physical, emotional, and spiritual needs and develop individualized care plans
  • Provide high-quality patient care and support to patients and families in need of palliative care services
  • Collaborate with the interdisciplinary team to develop and implement comprehensive care plans
  • Provide education and support to patients and families
  • Participate in quality improvement initiatives to enhance patient care and outcomes
Requirements:
  • Master's degree in Nursing and graduate of an accredited Nurse Practitioner program with specialization in palliative care
  • State licensure as an Advanced Practice Nurse (APN) and/or other designation as required by law
  • Minimum of two years recent and relevant nursing experience preferred
  • Effective communication and interpersonal skills
  • Excellent clinical assessment and analytical skills
