Job Title: APRN - Nurse Practitioner - Palliative Care

Job Summary:

The APRN - NP Specialty provides primary or acute healthcare services for specialty health conditions in specific populations in accordance with applicable scope and standards of practice and with the policies, values, and mission of the organization.

This position is for the inpatient Acute Palliative Care team. The APRN will work day shift and will rotate to Mount Carmel East, St. Ann's and Mount Carmel Grove City.

Key Responsibilities:

  • Provide primary or acute healthcare services for specialty health conditions in specific populations.
  • Work collaboratively with the interdisciplinary team to provide high-quality patient care.
  • Develop and implement individualized care plans to meet the unique needs of patients.
  • Provide education and support to patients and their families to promote health and well-being.
  • Participate in quality improvement initiatives to enhance patient outcomes and satisfaction.

Requirements:

  • Master's degree in Nursing and graduate of an accredited Nurse Practitioner program with specialization in area of practice.
  • State licensure as an Advanced Practice Nurse (APN), Nurse Practitioner (NP) and/or other designation as required by law.
  • Minimum of two years recent and relevant nursing experience preferred.
