About the Role

Key Responsibilities
  • Conduct detailed psychosocial assessments with community resources and provide supportive counseling to patients and families.
  • Offer crisis interventions to patients and families with psychosocial needs and facilitate care for high-risk patients.
  • Communicate and manage all aspects of discharge planning for patients, ensuring a timely discharge and providing appropriate connection with post-discharge care providers.
  • Educate patients, families, and physicians regarding post-acute options and address freedom of choice and financial concerns.
  • Meet and adhere to core social work competencies and regulations, promoting individual, professional growth and development.
  • Advocate for patients and families by providing intervention in cases involving abuse of any kind, empowering them to make autonomous healthcare decisions and access needed services within the healthcare system and community.
Requirements
  • Master's Degree in Social Work or Masters of Science in Social Administration (MSSA).
  • Current state licensure as a Social Worker, including LSW, LISW, LISW-S, or PC.
  • Basic Life Support (BLS) certification through the American Heart Association (AHA).
  • Experience in hospital social work, home care, and/or hospice social work, with knowledge of community resources and excellent communication and negotiation skills.
  • Ability to work independently and collaboratively with multidisciplinary teams for optimal outcomes.
