Job Description

As a Transplant Social Worker at Cleveland Clinic, you will play a vital role in the care of patients and families undergoing transplantation. Your responsibilities will include conducting psychosocial assessments, developing treatment plans, and ensuring timely discharges. You will work closely with interdisciplinary teams to provide comprehensive care and address social, psychological, financial, and behavioral barriers to successful medical outcomes.

Key Responsibilities

* Conduct comprehensive psychosocial assessments for patients and families undergoing transplantation
* Develop and implement treatment plans to address social, psychological, financial, and behavioral barriers
* Collaborate with interdisciplinary teams to ensure timely discharges and provide appropriate linkage with post-discharge care providers and resources
* Educate patients and families about post-acute options and address concerns related to freedom of choice and financial support
* Serve as a resource person and provide counseling and intervention related to treatment decisions and end-of-life issues
* Advocate for patient and family empowerment and independence to make autonomous healthcare decisions and access needed services within the healthcare system

Requirements

To be successful in this role, you will need a Master's degree in social work or a related field, current state licensure as a Social Worker, and experience in pre-acute and post-acute care. Excellent communication and negotiation skills are essential for working collaboratively with multidisciplinary teams to achieve optimal outcomes. Additionally, you will need to be flexible, open-minded, and adaptable to change.

Preferred Qualifications

While not required, hospital social work experience and knowledge of community resources are highly valued. Our ideal candidate will be passionate about delivering exceptional patient care and committed to advancing the field of transplant social work.
