**Job Summary:**

We are seeking a highly skilled Performance Improvement Specialist to join our team at Charter Care Health Partners. This role plays a critical part in ensuring the delivery of high-quality care to our patients.

About the Role:

  • Participate in professional growth and development, carrying out the CCHP Mission to achieve excellence in healthcare services.
  • Collaborate with the Director of Home Care and Rehab to coordinate department activities and support the tracking and trending of patient outcomes.
  • Develop and implement infection control programs and ensure compliance with regulations.
  • Evaluate clinical staff and assess their adherence to policies and procedures.
  • Perform ongoing orientation and education for clinical staff and participate in ongoing staff development programs.

Requirements:

  • Bachelor's degree in Nursing (BSN) preferred; Master's degree in Healthcare or Nursing is advantageous.
  • 4 years of full-time clinical nursing experience, including 2 years in a home health agency.
  • Certification in Wound Care is required; Ostomy certification is preferred.
  • Excellent interpersonal, oral, and written communication skills, as well as organizational and problem-solving abilities.
  • Able to educate and evaluate staff field performance, provide education as needed, and create care plans for ostomy and wound care patients.
