Job Summary:
Bayview Hunters Point Foundation for Community Improvement is seeking a skilled Case Manager II to provide comprehensive case management services to inmates in a correctional facility. The successful candidate will work closely with correctional staff, external service providers, and community resources to address the needs of inmates and facilitate their transition post-release.
Key Responsibilities:
- Conduct thorough assessments of inmates' needs, risks, and strengths upon intake.
- Develop individualized case plans that outline specific goals and interventions.
- Advocate for inmates' needs within the correctional facility and with external service providers.
- Provide counseling and support to help inmates cope with incarceration and prepare for reentry into society.
- Connect clients with essential resources, such as emergency shelter, food, healthcare, mental health services, substance abuse treatment, and employment support.
- Collaborate with clients to develop individualized service plans that address their unique circumstances.
- Regularly reassess and update service plans based on clients' progress and changing needs.
Requirements:
- Experience working with vulnerable populations, preferably in a correctional setting.
- Basic knowledge and understanding of homelessness issues, social services, and community resources in San Francisco.
- Familiarity with trauma-informed care, cultural competency, and client-centered approaches.
- Excellent interpersonal and communication skills to establish rapport, actively listen, and effectively communicate with clients, colleagues, and external stakeholders.
- Basic organizational and time management skills to handle caseloads, maintain documentation, and meet deadlines.
- Empathy and compassion to provide support and assistance to individuals experiencing homelessness.
- Problem-solving skills to identify client needs, navigate resources, and address barriers.
- Cultural sensitivity and the ability to work with individuals from diverse backgrounds.
Preferred Qualifications:
- Strong understanding of the criminal justice system and challenges faced by incarcerated individuals.
- Can demonstrate a commitment to professional development and enhance candidacy.
- Equivalent work experience acceptable in lieu of degree with a minimum of one (1) year direct experience in case management or related field.
- Clinical supervision toward licensure is available for those on a BBS licensure track if selected.
