Case Manager II Position

Bayview Hunters Point Foundation is seeking a highly skilled Case Manager II to provide comprehensive case management services to inmates in a correctional facility. The successful candidate will be responsible for assessing inmates' needs, developing individualized case plans, and coordinating access to essential resources.
Key Responsibilities:
- Conduct thorough assessments of inmates' needs, risks, and strengths upon intake.
- Develop individualized case plans that outline specific goals and interventions.
- Advocate for inmates' needs within the correctional facility and with external service providers.
- Provide counseling and support to help inmates cope with incarceration and prepare for reentry into society.
- Connect clients with essential resources, such as emergency shelter, food, healthcare, mental health services, substance abuse treatment, and employment support.
- Collaborate with correctional officers, healthcare providers, social workers, and community organizations to deliver comprehensive care.
Requirements:
- Experience working with vulnerable populations, preferably in a correctional facility setting.
- Basic knowledge and understanding of homelessness issues, social services, and community resources in San Francisco.
- Excellent interpersonal and communication skills to establish rapport, actively listen, and effectively communicate with clients, colleagues, and external stakeholders.
- Empathy and compassion to provide support and assistance to individuals experiencing homelessness.

- Competitive salary range: $67,000.00 - $73,000.00 (Annually)
