Job Description

Bayview Hunters Point Foundation is seeking a skilled Case Manager II to join our team. As a Case Manager II, you will be responsible for providing comprehensive support services to individuals and families experiencing homelessness. Your primary objective will be to empower clients to overcome barriers and achieve self-sufficiency.

Key Responsibilities:

  • Conduct comprehensive assessments of individuals experiencing homelessness to identify their needs, strengths, and goals.
  • Develop individualized case plans in collaboration with clients, addressing their immediate and long-term needs, such as housing, employment, healthcare, and social support.
  • Connect clients with appropriate resources and services, such as housing programs, mental health services, substance abuse treatment, job training, and legal assistance.
  • Advocate for clients' rights and needs within the shelter and in the community, including liaising with social service agencies, government entities, and community organizations.
  • Provide immediate support and intervention during crisis situations, such as mental health crises, substance abuse relapses, domestic violence incidents, or medical emergencies.
  • Regularly monitor clients' progress, document interactions, and maintain accurate and confidential case records.
  • Offer emotional support, guidance, and counseling to clients, helping them develop coping skills, problem-solving abilities, and positive relationships.
  • Facilitate workshops and group sessions on life skills, including budgeting, job readiness, communication skills, and conflict resolution.
  • Collaborate with other case managers, shelter staff, and external service providers to ensure coordinated and holistic care for clients.
  • Assist in evaluating the effectiveness of the shelter's programs and services, providing feedback and recommendations for improvement.

Requirements:

  • Experience working with vulnerable populations, preferably in a case management role.
  • Basic knowledge and understanding of homelessness issues, social services, and community resources in San Francisco.
  • Excellent interpersonal and communication skills to establish rapport, actively listen, and effectively communicate with clients, colleagues, and external stakeholders.
  • Empathy and compassion to provide support and assistance to individuals experiencing homelessness.
  • Problem-solving skills to identify client needs, navigate resources, and address barriers.
  • Cultural sensitivity and the ability to work with individuals from diverse backgrounds.
