Job Description

At St. Vincent De Paul Village, we are seeking a skilled Case Manager II to lead our case management team and provide intensive support to clients. As a Case Manager II, you will be responsible for developing professional relationships with clients, assessing their needs, and creating individualized case plans to promote self-sufficiency.

Key Responsibilities:

  • Provide ongoing support to clients, including assessing, evaluating, and coordinating services, crisis intervention, and referrals to community resources.
  • Manage a caseload of clients and enter, monitor, and update client database.
  • Prepare and present client cases and participate in multidisciplinary team meetings.
  • Communicate and advocate with landlords to support clients in maintaining their housing.
  • Plan, coordinate, and facilitate client orientation, meetings, and events.
  • Transport clients to medical appointments, income-related appointments, and other services critical to client case plan progress and housing stability.
  • Support team in identifying valuable education topics, strategies, and community resources, and occasionally lead trainings.
  • Develop and implement a plan to provide social events and community engagement opportunities to increase resident engagement.
  • Collaborate with service providers to develop a site program schedule for residents.
  • Assist supervisors with onboarding process of new hires.
  • Stand in the absence of the Supervisor of Case Management and assist with file reviews.
  • Complete assigned training and policies on time.

Qualifications:

  • Bachelor's Degree in healthcare, psychology, sociology, social work, public administration, or a related field, and one year of applicable job experience, or
  • Associates Degree with a focus on healthcare, psychology, sociology, social work, public administration, or a related field, and two years of applicable job experience, or
  • High school Diploma or GED and a minimum of two years of lived experience, and at least two years of applicable job experience.
  • Demonstrated basic understanding of principles of Housing First, Trauma Informed Care, Conflict Resolution, Motivational Interviewing, Low Barrier Operations, and Prevention and Diversion practices.
  • At least two years' experience in case planning and/or completing assessments in a social service setting.
  • Basic user of MS Office.
  • Ability to pass fingerprinting and background check upon employment.
  • Must have a valid California Driver's License and be able to qualify for insurance coverage.
  • Participate in an annual Tuberculosis screening and/or other screenings when necessary.
