Job Description

We are seeking a dedicated and compassionate Care Coordinator to support our team. The successful candidate will play a crucial role in ensuring our clients receive the best possible care. Key responsibilities include:

  • Managing day-to-day office and field operations to ensure quality assurance of services
  • Answering phone calls and conducting intakes, handling scheduling issues, and client referrals
  • Performing caregiver introductions, coordination, and supervision of client services
  • Assisting with scheduling shifts by matching caregiver qualifications and availability to client needs
  • Visiting prospective clients to introduce Home Helpers Home Care and providing ongoing support as needed
  • Staying current with changing technology, including software programs
  • Upholding, supporting, and promoting all company policies and procedures
  • Maintaining accurate clinical records in compliance with local, state, and federal laws
Qualifications

The ideal candidate will possess:

  • Experience in home care, social work, or a related field
  • Strong organizational and multitasking skills
  • Excellent communication and interpersonal skills
  • Ability to work independently and as part of a team
  • Compassionate and patient-centered approach to care
  • Familiarity with care coordination and home care services
  • Basic computer skills, including proficiency with Microsoft Office Suite
Education/Experience

The successful candidate will have:

  • An Associate degree (A.A.) or equivalent from a two-year college or technical school, and 2 years of related healthcare/home care industry experience and/or training
  • Successful management experience
  • Proficient skills in Microsoft Office and scheduling software
  • Experience with Wellsky preferred
  • Excellent organizational, communication, interpersonal, and leadership skills, as well as a positive and professional image
Certifications, Licensure, & Registration

The ideal candidate will possess:

  • A valid driver's license and adequate auto insurance
  • CNA certification preferred
