Job Title: Self-Determination Program Coordinator

**Job Summary:**

We are seeking a highly skilled and dedicated professional to join our team as a Self-Determination Program Coordinator at the San Andreas Regional Center. As a key member of our organization, you will play a vital role in providing and coordinating services to consumers in accordance with the provisions of the Lanterman Developmental Disabilities Services Act and policies established by our Board of Directors.

**Key Responsibilities:**

  • **Provide Guidance and Support:** Work primarily with individuals in the Self-Determination program, providing guidance and support to help them achieve their goals.
  • **Stay Up-to-Date with Regulations:** Stay current with all regulations, policies, and principles relating to Self-Determination, ensuring that services are delivered in accordance with the latest guidelines.
  • **Assist with Budgeting and Planning:** Assist consumers and families in creating budgets and spending plans for Self-Determination, ensuring that their financial needs are met.
  • **Attend Meetings and Workshops:** Attend meetings and workshops related to Self-Determination, staying informed about the latest developments and best practices.
  • **Develop Person-Centered Plans:** Develop and implement Person-Centered Plans, working closely with consumers and their families to identify and achieve their goals.
  • **Conduct Assessments and Gather Data:** Conduct assessments and gather data to inform service planning and delivery.
  • **Provide Counseling and Advocacy:** Provide counseling and advocacy services to consumers and families, ensuring that their needs are met and their rights are protected.
  • **Identify Areas of Unmet Need:** Identify areas of unmet need and develop strategies to address them.
  • **Perform Fiscal and Accountability Tasks:** Perform tasks related to fiscal and accountability requirements, ensuring that services are delivered in a fiscally responsible manner.
  • **Develop and Facilitate Support Groups:** Develop and facilitate support groups for consumers and families, providing a safe and supportive environment for them to connect and share their experiences.
  • **Maintain Accurate Records:** Complete all required caseload documentation, ensuring that records are accurate and up-to-date.

**Requirements:**

  • **Bachelor's Degree:** Bachelor's Degree in social work, rehabilitation counseling, or a related field from an accredited college or university.
  • **Experience:** Four years of experience in a related field, with a strong understanding of the principles and practices of social work.
  • **Knowledge of Self-Determination:** Knowledge of the Self-Determination program, including regulations, policies, and procedures.
  • **Interpersonal Skills:** Ability to relate in a constructive and enabling manner to a wide variety of individuals and organizations.
  • **Communication Skills:** Ability to establish and maintain rapport with clients and their families, as well as with colleagues and supervisors.
  • **Reporting and Record-Keeping:** Ability to prepare and present oral and written reports, as well as maintain accurate and timely case records.

**Preferred Qualifications:**

  • **Experience Working with Individuals with Developmental Disabilities:** Experience working with individuals with developmental disabilities and their families.
  • **Knowledge of Community Resources:** Knowledge of community resources and services, including those related to Self-Determination.
  • **Collaboration and Communication:** Ability to work independently and as part of a team, with a strong commitment to collaboration and communication.
