Key Responsibilities:

  • Establish person-centered goal-oriented, culturally relevant, and logical steps to ensure individuals receive services in a supportive, effective, efficient, timely, and cost-effective manner.">
  • Care coordination includes case management, disease management, discharge planning, transition planning, and addressing social determinants of health and integration into community.">
  • Assess, plan, and implement care strategies that are individualized by the individual and directed toward the most appropriate, least restrictive level of care.">
  • Identify and initiate referrals for social service programs, including financial, psychosocial, community, and state supportive services.">
  • Ensure the care plan/plan of service includes covered services, EPSDT, in lieu of services, value-added benefits, informal supports, and addresses social determinants of health.">

What You'll Need:

To be successful in this role, you'll need:

">

Education:

">

A Bachelor's Degree (or higher) in social work, rehabilitation, nursing, psychology, special education, gerontology, or a related human services area.

">

Experience:

">

1+ years of experience working with individuals with long-term care needs and 1+ years of experience working within the community health setting in a healthcare role.

">

Skills:

">

You'll need strong communication and customer service skills, both in person and via phone, as well as the ability to work independently and maintain good judgment and accountability.

Salary:

">

This role offers a competitive salary range of $62,500 - $85,000 per year, depending on experience.
