Key Responsibilities:
- ">
- Establish person-centered goal-oriented, culturally relevant, and logical steps to ensure individuals receive services in a supportive, effective, efficient, timely, and cost-effective manner.">
- Care coordination includes case management, disease management, discharge planning, transition planning, and addressing social determinants of health and integration into community.">
- Assess, plan, and implement care strategies that are individualized by the individual and directed toward the most appropriate, least restrictive level of care.">
- Identify and initiate referrals for social service programs, including financial, psychosocial, community, and state supportive services.">
- Ensure the care plan/plan of service includes covered services, EPSDT, in lieu of services, value-added benefits, informal supports, and addresses social determinants of health.">
What You'll Need:
">To be successful in this role, you'll need:
">Education:
">A Bachelor's Degree (or higher) in social work, rehabilitation, nursing, psychology, special education, gerontology, or a related human services area.
">Experience:
">1+ years of experience working with individuals with long-term care needs and 1+ years of experience working within the community health setting in a healthcare role.
">Skills:
">You'll need strong communication and customer service skills, both in person and via phone, as well as the ability to work independently and maintain good judgment and accountability.
">Salary:
">This role offers a competitive salary range of $62,500 - $85,000 per year, depending on experience.

Location:
">This position is based in Reno, Ford, Sedgwick counties, and surrounding areas, with local travel up to 75% and mileage reimbursement at the current government rate.
